I acknowledge and consent to the prescribing Specialist, communicating with my GP details of my consultation and medication prescribed.

I acknowledge and consent to the prescribing specialist discussing my details at the in-house MDT level and (if appropriate) potentially with an external expert panel (anonymised).

I understand and accept that the Specialist or the Clinic will neither provide nor dispense medical cannabis directly but may provide a prescription.

I understand and accept that the Specialist or the Clinic may not provide a prescription for Medical Cannabis at the completion of my appointment if the Specialist deems it inappropriate or not suitable.

I understand and accept that the Specialist or the Clinic will not be providing or discussing information with me regarding any other way of obtaining medical cannabis other than via an approved medical pharmacy.

I understand that the Specialist will assess me for the use of medical cannabis and will make their recommendations based in part on the medical information I provide. I understand that it is my responsibility to provide as much accurate, correct and up to date information as possible during my assessment and on my medical questionnaire about my past and current medical conditions, and to ensure that no misrepresenting information is submitted, in order for me to receive the best possible recommendation for use of medical cannabis for my medical condition.

I agree to only use medical cannabis for the treatment of my medical condition as discussed with and agreed by the Specialist. I agree that I shall not use medical cannabis for recreational or non-medical purposes. I understand that, depending on dosage and administration, impairment of my judgement and coordination (amongst other things) can last over 24 hours following last usage. I will ensure that I inform the Specialist of my occupation.

I understand that using cannabis while under the influence of alcohol is not recommended. Additional side effects may become present when using both alcohol and cannabis. Should I have concerns about this I will discuss this with the Specialist or the Clinic.

I understand that medical cannabis is prescribed in this clinic on an unlicensed basis. The risks, benefits and drug interactions of medical cannabis are not fully understood. If I am taking any other medication or undergoing treatment for any medical condition, I understand I should consult with my GP and the Specialist before commencing to use medical cannabis, and that this information must be provided during my initial medical assessment at the Clinic.

I am aware that it is not advised to use medical (or otherwise) cannabis when pregnant or breastfeeding and will advise the Specialist or the Clinic if I become pregnant or breastfeed.

Should I commence treatment with medical cannabis, but decide I no longer wish to continue with such treatment, I will follow my Specialist’s guidance on discontinuing treatment and seek guidance from them or my GP should I need to. Should I experience any withdrawal symptoms, I will seek medical advice.

I understand that should I have any questions about treatment at any time, I should contact the Specialist or the Clinic or my GP.

I understand that the information I have been asked to provide to the Specialist or the Clinic is for the treatment of the medical condition for which I wish to access prescribed medical cannabis. I understand that if I have not accurately and completely disclosed the requested information, it may adversely impact the specialists’ ability to treat my condition and recommend the appropriate medical cannabis treatment.

I authorise Indra Clinic and it's pharmacy partners to access my Summary Care Record and any clinical letters related to any condition listed on my SCR.

I consent to be contacted regarding my treatment via email, SMS or phone.

I confirm that I understand that there are costs associated with the clinic consultations (after the first free consultation) and any medications that may be prescribed.

I understand that it will be necessary for the Specialist to process my personal data in order to provide treatment of the medical condition for which I wish to gain access to prescribed medical cannabis and I consent to this in accordance with the terms of the Clinic privacy policy which can be found via http://indra.clinic
